Population Health Specialist

Clinical Programs

The Population Health Specialist- Clinical Programs assists the Population Health Manager with the day-to-day operations and maintenance of the Transitional Care Management program.  The desired candidate will assist in the efficiency of delivery of care, supports the professional role of licensed team members and provides ongoing support and assistance to the Population Health Manager.

 

Essential Functions and Duties

  • Acts as a liaison between patients, patients' relatives and the healthcare organization
  • Assist the TCM Team in identifying hospitalized patients across the hospital settings on a daily basis
  • Obtain pertinent records for all hospitalized patients according to the TCM protocol, prior to follow-up visit with Primary Care Provider
  • Facilitate ER follow-up care to appropriate staff member
  • Demonstrate effective teamwork, prioritization, communication and organization skills
  • Assist in complex care coordination for the following: High ED utilizers, poorly-controlled diabetes, etc.
  • Coordinate referrals to other health care providers
  • Support evolving clinical programs as needed

  • Detail oriented while maintaining a high level of efficiency and strong time management skills
  • Experience with and proven ability to learn various software applications

 

Education and Experience

  • High School Diploma Required
  • Knowledge of Medical Terminology required
  • Medical Assistant Certification Required
  • Proficiency in Microsoft Office (Excel, Word, Outlook)
  • Experience with Software Applications/Electronic Medical Record typically used in a medical office

Physical Requirements and Working Conditions

  • Sedentary work, requiring lifting up to 10 lbs.
  • Repetitive motion associated with operating a computer and other office equipment
  • Inside, climate controlled working conditions
