Overview:

Reporting to the Director of Operations, the Patient Referral Manager (PRM) is responsible for assisting patients with scheduling new and follow-up appointments with primary care and specialty practices.

 

Essential Functions and Duties:

  • Acts as the coordinator of patient care for referrals received from physicians' offices
  • Aligns physician referrals with patient needs to schedule patient appointments with the appropriate referred provider
  • Communicates with the patient in a timely manner to discuss appointment preferences
  • Processes and schedules appointments while ensuring complete and accurate registration, including but not limited to patient demographic and current insurance information
  • Communicates with and verifies appointment with referring practice
  • Communicates, problem solves, and prioritize issues with patients and providers
  • Involved in program implementation, ongoing support and maintenance of processes and policies and technologies as they relate to physician office activity
  • Develops and maintains contacts with specialists and ancillary services to ensure referral process meets the needs of their practice
  • Verifies eligibility, dates of coverage and benefits for specialty services
  • Communicates approval or denial of referral authorization to patient and referring provider
  • Conducts reminder and follow-up phone calls with provider offices and patients regarding appointments as scheduled
  • Obtains an understanding of the organizational and departmental systems of the clinic
  • Establishes and maintains patient and provider/personnel relationships
  • Communicates patient complaints or concerns to the appropriate manager or service line leader
  • Shares appropriate results/reports with physicians and office staff to gauge value and effectiveness of referral process for patients

  • Communicates appropriately and clearly with departmental management, co-workers and callers and exhibits willingness to become educated to all department scheduling processes
  • Demonstrates exceptional judgement and tact in handling routine and unexpected interactions with patients and physicians
  • Demonstrates excellent judgement in handling situations not covered by written or verbal instructions
  • Able to manage multiple priorities and manage stress appropriately
  • Demonstrates a commitment to the mission, core values, and goals of Granger Medical Clinic and its healthcare delivery including the ability to deliver exceptional patient focus with quality, compassion and respect.

 

Education and/or Experience

  • Associate's degree or equivalent from a two-year college or technical school and two years' experience in a healthcare setting; or equivalent combination of education and experience.
  • Knowledge and understanding of basic medical terminology
  • Experience with Microsoft Office Applications
  • Experience with electronic medical records programs
  • Ability to work in a fast-paced and evolving healthcare environment
  • High degree of problem solving and critical thinking skills

 

Physical Requirements and Working Conditions:

  • Repetitive motion associated with operating a computer and other office equipment
